An update to a breach previously noted on this blog.
Reynaldo Estrada was sentenced yesterday to 51 months in prison for stealing the identity information of numerous patients’ as part of a tax fraud scheme. The patient information theft occurred while Estrada was working at Crozer-Chester Medical Center’s Environmental Services Department in Upland and at Community Hospital in Chester between October 2010 and October 2011.
In addition to prison, Estrada was also sentenced to three years of supervised release following prison, and ordered to pay $409,779 in restitution, and a $300 special assessment.
Two others, Rafael Henriquez Polanco and Yanira Lopez, who paid Estrada for the stolen treatment authorization forms containing patients’ names, addresses, dates of birth, and Social Security numbers, were charged separately for filing 144 fraudulent income tax returns claiming refunds in excess of $1.7 million.
