The Laborers’ Health & Welfare Trust Fund for Northern California has experienced a recent HIPAA security incident involving eligibility information.
The Laborers’ Funds Administrative Office, who administers the Laborers’ Health & Welfare Trust Fund for Northern California, recently generated and distributed IRS Form 1095-B to plan participants. On February 17, 2016, the Fund Office discovered that due to a computer error, a portion of the Form 1095-B documents were improperly processed and personal information of some participants and beneficiaries were sent to other plan participants and beneficiaries.
According to the Fund Office’s internal investigation, the 1095-B forms contained some protected health information, including full names of participants and dependents, social security numbers and health plan coverage information. The Fund Office has notified participants and provided credit monitoring services to all those participants and beneficiaries affected. Additionally, steps to strengthen training policies and procedures have been made. The Fund Office has also instituted stronger security measures to guard against future mishaps.
