Updating some previously reported breaches

HHS has updated its breach tool, which allows me to update a few previously reported breaches:

The Texas Health Presbyterian Hospital Flower Mound breach, involving a stolen a laptop, affected 10,345.

RxAmerica, named in mailing error glitch that affected Accendo Insurance and Molina Medicare, was also named by Windsor Health Plan in Tennessee in their report to HHS. According to the breach tool, 1,378 of Windsor’s enrollees were affected. Significantly, perhaps, they list a March 1 date for their breach, so it’s not clear whether this is part of the original incident or yet another incident for RxAmerica.  RxAmerica lists no corporate contact for media and a call to Caremark corporate has not yet been returned. (See update, below)

The Futurity First Insurance Group breach that affected 582 Ohioans enrolled in United Healthcare Medicare, also affected 705 members of Mutual of Omaha Insurance Company, 1,631 enrollees of United of Omaha Life Insurance Company, and 3,994 enrollees of United Health Group Health Plan in Minnesota. The HHS reports indicate that the loss occurred on July 28, not June 28 as reported in the media.

The Lexington (KY) VAMC breach affected 1,432 and occurred on May 23.

Update: I have been informed by a corporate spokesperson that there was only one incident involving RxAmerica and that the Windsor report was part of that incident, despite the apparent discrepancy in the reported incident dates.
