Laptop with Orthopedic Physician Associates' patient data stolen from car

NOTICE OF SECURITY BREACH ORTHOPEDIC PHYSICIAN ASSOCIATES

On April, 1, 2013, a laptop and ten patient files were stolen during a car break-in. The patient files were subsequently recovered. However, information regarding some patients of Orthopedic Physician Associates, a division of Proliance Surgeons, may have been compromised by this theft. Sensitive information, including name, address, telephone number, social security number, name of provider, health insurance information and the reason for the patients appointment was included in emails stored in the laptops cache file. Please call 1-888-608-7344 or email [email protected] if you believe you may have been affected by this incident or for further information about this incident. It is the mission of Proliance Surgeons, Inc., P.S. to provide quality care to every patient, which includes respecting privacy and maintaining the confidentiality of health information. Proliance Surgeons is in the process of updating its security practices, including retraining its employees and making software changes to minimize the impact of thefts to our patients. This notice of a potential security breach is provided in accordance with the Health Insurance Portability Accountability Act (HIPAA).
