Today's HHS update included 16 incidents we already knew about

Working through the latest updates to HHS’s breach tool, the following are  incidents where we already knew something about the breach.  Once again, some of these entries refer to breaches that occurred in 2012, and I have no idea why HHS didn’t post these publicly before now. Links are to previous coverage on this blog and in a few cases, I’ve annotated the list to point out new information or conflicting information.

  1. St. Joseph Health System
  2. Cabinet for Health and Family Services, Department for Community Based Services (Protection and Permanency)
  3. Ochsner Health System
  4. City of Chicago
  5. Kaiser Foundation Hospital- Orange County
  6. North Carolina Department of Health and Human Services – Division of State Operated Health Care Facilities
  7. SSM St. Mary’s Health Center breach involving Saint Louis University was reported as affecting 1,300. Coverage elsewhere had indicated 3,000.
  8. Good Samaritan Hospital (California). This entry appears to be a duplicate as it was already in their database in October.
  9. Bronx-Lebanon Hospital breach involving Professional Transcription Services was reported as affecting 10,930.  This appears to be a duplicate as the incident was previously entered in their database as the Martin Luther King Jr. Health Center incident. Strangely, the previous entry showed 37,000 while this entry shows 10,930 affected.
  10. Seton Healthcare Family
  11. SIU HealthCare
  12. UniHealth Source
  13. Horizon Healthcare Services, Inc., doing business as Horizon Blue Cross Blue Shield of New Jersey, and its affiliates“
  14. Florida Digestive Health Specialists
  15. Fairfax County, Virginia
  16. Tennova Cardiology. The HHS entry reveals that Colby DeHart was the business associate involved.

In the next post, I’ll try to research the remainder of today’s update, which consists of entries we didn’t know about until now.
