Ladies First Choice, Inc. in Florida reported to HHS that 2,365 were affected by an incident on January 1, 2013. The incident was coded as “Theft, Unauthorized Access/Disclosure,” with the location of the information listed as “laptop.”
On investigation, I don’t think that the incident occurred on January 1, 2013, but rather that the oldest customer data may go back to that date. Unfortunately, their notification is not clear about when the theft occurred and when it was first discovered.
Ladies First Choice deals in post-mastectomy products.
On April 11, 2014, they posted a notice to their web site that explains that a former employee took customer information to use at her new job with a competitor. That information may have included customers’ contact information, order history, medical care provider information, and Medicare or Social Security numbers.
