WHIO reports:
Wright-Patterson Air Force Base officials confirm they are investigating an incident at the Department of Air Force 88th Medical Group involving a binder containing names and Social Security numbers that was misplaced from their blood donor program.
The 88th Medical Group sent more than 3,000 letters notifying donors that a data binder with the information was left unattended in a locked room over the Labor Day weekend, base officials said. Human error was blamed, they said. The blood drive was Aug. 27.

To quote the famous data breach expert, Yogi Berra, this breach is deja vu all over again. In 2012, this blog noted an identical breach that resulted in the notification of 3,800 blood donors. WHIO mistakenly reports that breach as having occurred in August 2013, but it was 2012.
