Donovan Vincent and Richard J. Brennan report that an error involving Ontario’s Social Assistance Management System resulted in 720 assistance recipients having their social insurance numbers and benefits amount for 2014 sent to their former trustee instead of to them.
The Ministry of Community and Social Services confirmed that a breach involving the T5 forms occurred on February 16 when the T5 forms were mailed out.
How the breach occurred is not totally clear from the available information:
The province is blaming the privacy breach, involving misdirected T5 forms, on an “interface’’ that pulled data from the troubled Social Assistance Management System (SAMS), which manages hundreds of thousands of Ontario Works and Ontario Disability Support Program caseloads.
