Hundreds of patients’ test results went missing last month from a Community Health Network office, the hospital network announced Friday.
In February, the staff at Physician Network Practice, located in the 2000 block of North Shadeland Avenue, noticed a binder with results had disappeared.
The binder contained more than 600 hemoglobin and lipid profile tests for services at the location between January and September of 2014. Staff immediately began looking for the binder, but as of Friday had not found them.
Community Health officials said the results were limited to name, date of service, test type and test results. No Social Security numbers or addresses were included in the binder.

In 2013, CHN disclosed that an employee of Community Health MedCheck in Speedway might have accessed the computerized medical records of up to 180 people, and may have taken their Social Security numbers, dates of birth, and credit card numbers.
