Here’s one you won’t see on HHS’s public breach tool, because it’s less than 500 patients.
Harford Surgical Associates in Bel Air, Maryland notified the Maryland Attorney General’s Office that due to a mailing error on October 16, 2014, patients received up to three other patients’ billing statements. The statements included the patients’ names, addresses, telephone numbers, Harford account numbers, and billing information.
Harford recognized its error the next day and within three days, sent out new statements with an attached letter asking patients who received others’ statements to either return the statements to the office or call the office.
