Sunbury Plaza Dental in Ohio has notified HHS of a breach that impacted 7,784 patients.
Their public notice explains that on May 25, they were notified by local law enforcement that their secured storage unit containing business records had been burglarized sometime between March 10th and
March 20, 2016.
We were not aware of any breach until being notified by law enforcement. Law enforcement informed us that the individuals who committed this crime gained access to the secured storage unit and did remove some of the patient records. However, the majority of the patient records in storage were not removed from the storage unit.
The patient records contained personal identifying information, including name, address, date of birth and social security number. The records also included personal health information. The records did not include credit card or payment information.
Those notified were offered identity theft protection services with Kroll.
