From their substitute notice:
On August 15, 2016, Mercy Hospital & Medical Center discovered that some medical billing information for a total of 547 patients was potentially exposed as a result of a third-party billing service’s loss of Mercy’s documents from their offices. The information exposure occurred on August 15, 2016. We have taken the necessary steps to notify and protect those who have been impacted. However, a small number of patients, approximately ten, are unknown. Information potentially exposed included a patient’s name, health insurance plan and member identification number, and billing account number. Additionally, it may also include dates of service and charge codes and procedure codes. Mercy Hospital and Medical Center takes our responsibility to safeguard personal information very seriously, and we deeply regret any inconvenience or concern this situation may have caused our patients. At this time, we are glad to report that Mercy has received no reports that information lost has been accessed or used improperly. If you have reason to believe you are affected by this incident, you may call Mercy Hospital Medical Center representatives at (312) 567-2028, Monday thru Friday, 8:00am to 5:00pm Central.
