RI: DHS glitch results in 1095-B tax forms being sent to wrong parties

A new glitch in the computer system at Rhode Island’s Department of Human Services resulted in more than 1,000 people receiving tax forms with the wrong information, putting their personal information at risk, state officials said Monday.

The system, known as UHIP, has been plagued by problems since its launch in September and caused thousands of people to see their benefits cut off or delayed.

In the latest glitch, tax form 1095-B was sent out to approximately 1,100 DHS customers who use the Medicaid program for health care, but was mailed to the wrong person and in some cases displayed the wrong details, including dates of birth and partial Social Security numbers.

    My daughter received this form and we have never applied for any assistance. We would like to know how the state got her personal ino if we never filled out any forms.
