Valley Family Medicine (VFM) in Staunton announced Friday that there was a breach of protected health information for some of its patients in mid-July.

Two employees printed and misused a mailing list of 8,450 patient names and addresses. One of those employees then used that list to make postcard notifications informing certain patients that they were starting a new practice and inviting the patients to visit, according to the release.
