Oregon Eye Specialists, PC notifies patients after email breach

PORTLAND, Ore., Oct. 8, 2021 /PRNewswire/ — Oregon Eye Specialists, PC announces a data incident that may have impacted a limited number of individuals’ information.

On August 10, 2021, Oregon Eye Specialists, PC identified unusual activity in an email account. An investigation determined that an individual accessed certain accounts between June 29, 2021 and August 31, 2021. Therefore, Oregon Eye Specialists, PC is reviewing the contents of the email accounts to determine the types of information contained therein.  Although there is no evidence of actual or attempted misuse of information, they are providing notice to potentially impacted individuals.

The type of information potentially impacted includes:  date of birth, date of service, medical record number, financial account information, and/or health insurance provider name/policy number.

In response to this incident, Oregon Eye Specialists, PC changed email account passwords and is implementing additional security measures. They are offering potentially impacted individuals access to credit monitoring and identity protection services.

To obtain more information about this incident or enroll in these services, individuals should contact their dedicated assistance line at 855-675-3076, Monday through Friday (excluding holidays), 6 am – 6 pm PST.

Individuals are encouraged to remain vigilant against incidents of identity theft and fraud by reviewing credit reports/account statements and explanation of benefits forms for suspicious activity and to detect errors.  Individuals may also place a fraud alert or credit freeze on their credit report by contacting the credit reporting agencies: TransUnion 1-800-680-7289; Experian 1-888-397-3742; Equifax 1-888-298-0045. Instances of known or suspected identity theft should also be reported to law enforcement, the state Attorney General, and the FTC.
