UnitedHealthcare has notified more than 1,000 members of a Deere & Co. employee benefit plan that 2009 claims summary statements were inadvertently sent to the wrong members last month.
The insurance provider said the isolated database error affected 1,097 members in a single mailing. The summary statements included costs, dates of service, categories of services and physician names.
According to UnitedHealthcare spokesman Greg Thompson, the error combined members’ names and information on a single summary. No diagnostic information or social security numbers was involved, he added.
