MobilexUSA today formally notified affected Indiana patients of a possible breach of protected health information. As a precautionary measure, the company has retained on behalf of affected patients the services of Kroll Inc., a global leader in risk mitigation and response with extensive experience helping people who have sustained an unintentional exposure of confidential data.
On or about May 1, 2014, Mobilex mailed a package from its office in Indiana to a corporate office in Pennsylvania. As is its customary practice, Mobilex mailed the package via a well-recognized overnight mail carrier and obtained a tracking confirmation. The package contained patient records related to x-ray services that Mobilex provided between April 1, 2014 and April 30, 2014.
On or about May 15, 2014, Mobilex contacted the carrier to inquire about the package location. The carrier informed the Company that they could not immediately locate the package and noted that they had initiated a “lost package investigation.” After numerous emails and phone calls to inquire about the package, on or about June 13, 2014, the carrier confirmed that the package was lost and could not be found. To date, the package has not been located. The breach has been reported to the U.S. Department of Health and Human Services.
On behalf of affected patients, Mobilex has secured the services of Kroll to provide identity theft protection services at no cost to affected patients for 1 year. Affected patients will receive individual letters containing further details including information on how to access credit monitoring. Patients who do not receive letters and feel they may have been impacted can call 1-855-781-0034, 8 a.m. to 5 p.m. (Central Time), Monday through Friday.
