I had noted this breach on PHIprivacy.net, but since there’s an update today, I’ll repeat the original information here.
On March 6, Erin Allday reported that a former UCSF doctor who was employed at San Francisco General Hospital from 2005 to 2013 had wrongfully removed copies of patient records from the medical center. Allday reported:
UCSF reported the security breach to the San Francisco Department of Public Health on Feb. 13. The incident is under investigation and authorities don’t yet know how many patients were affected or when the files were taken.

Today, the incident was added to HHS’s public breach tool. The entry reveals that 2,500 patients were affected by, or notified of, the incident.
