Loyola University Medical Center Notice of Privacy Breach

This notice is provided by Loyola University Medical Center concerning a privacy incident affecting some of its patients. On April 5, 2016, Loyola University Medical Center learned that a project that occurred in February, 2016 undertaken to acquire accurate addresses may have resulted in the release of billing statements to certain unverified addresses. The billing statements included name, account number, date of service, balance due and a procedure code with general description of service. The billing statements did not contain social security numbers or any other financial information.

Loyola University Medical Center has taken steps including but not limited to adding more data elements in the address search query and calling certain patients prior to mailings to revise its address research projects to ensure billing statements containing patient information are being mailed to accurate addresses. Loyola University Medical Center sincerely apologizes for the inconvenience and concern this incident may cause. If you have reason to believe you are affected by this privacy incident, you may call Loyola University Medical Center representatives at 1-800-424-4840 Monday thru Friday, 8 a.m. to 6 p.m. Central.
