Command Marketing Innovations Reports Printing Error Impacting Horizon Blue Cross Blue Shield of New Jersey Members

The vendor involved in the recent breach reported on this site involving Horizon Blue Cross Blue Shield of New Jersey has now issued its own press release:

GARFIELD, N.J., Nov. 23, 2016  — Command Marketing Innovations (CMI), a vendor performing print work for Horizon Blue Cross Blue Shield of New Jersey (Horizon BCBSNJ), announced today that on November 2, 2016, it discovered a printing error that caused certain Horizon BCBSNJ members and providers to receive Explanation of Benefits (EOB) statements and Explanation of Payment (EOP) statements that included information intended for a different Horizon BCBSNJ member or provider.  Horizon BCBSNJ was made aware of the incident that same day and EOB and EOP processing was immediately suspended.

The print error was determined to be related to a change in the printing process made by CMI. CMI has implemented corrective actions to restore compliance with Horizon BCBSNJ’s strict quality control and privacy standards and assure accurate performance going forward.  Only EOBs and EOPs mailed on October 31, November 1 and November 2, 2016 were impacted. Electronic EOBs and EOPs available via Horizon BCBSNJ’s Online Services were not impacted. CMI apologizes to Horizon BCBSNJ’s impacted members for this error.

While no Social Security Numbers, financial information, addresses or dates of birth were included on the statements, EOBs and EOPs may include member name, member ID number, claim number, date of service, limited description of services, service codes or provider/facility name. Horizon BCBSNJ will monitor impacted members’ accounts for any potential fraudulent submission of medical claims. Corrected EOBs and EOPs will be reissued within the next week and notifications of the error will be mailed to impacted Horizon BCBSNJ members.

Horizon BCBSNJ members with questions about this matter may call Horizon BCBSNJ at 1-800455-9864, Monday through Wednesday and Friday, between 8 a.m. and 6 p.m., Eastern Time (ET), Thursday, between 9 a.m. and 6 p.m., ET and Saturday, between 9 a.m. and 5 p.m., ET.
