WLS reports:
The Illinois Department of Healthcare and Family Services says information on about 3,100 clients in Cook County may have been released.
It says a managed care contractor accidentally sent client ID cards to the wrong addresses.
The personal information on the Family Health Network ID card included member name, state issued Medicaid number and birth date.
The agency is trying to identify everyone affected and is issuing new ID cards.
The contractor was not named in this coverage nor in coverage in another news outlet, and I cannot find any notice on the state agency’s web site as of the time of this posting.
