Harel Chiropractic Clinic notified 3,000 patients Monday that some patient personal information may have been breached on Nov. 4, 2014, by an employee and a contract chiropractor. Clinic officials discovered the breach on Nov. 20.
Clinic officials say the employee and contract chiropractor had access to patient information as part of their normal job duties; they resigned shortly after the breach.
Clinic officials say information—including patients’ names, addresses, phone numbers, email addresses, birthdays, Social Security numbers and insurance information—may have been taken. No credit card, bank account, or financial information was accessed, they say.
