KDLT reports that Sioux Falls VA Health Care System is notifying veterans that their personal information may have been compromised in a potential breach of privacy:
Sioux Falls VA Officials say on June 4, 2015 a list of patients was reproduced whose February 2015 prescription fill documents cannot be located.
Officials say the fill documents were first discovered missing in mid-April, and a thorough search and investigation was conducted.
Read more on KDLT. I must confess that I do not totally understand the description of the breach, as reported:
- What information is included in prescription fill documents that would require the VA to offer credit monitoring services? Do they include full SSN?
- How many veterans were notified?
- If the list was first discovered missing in April (and how did they discover that?), why wasn’t this incident included in the VA’s April report to Congress – or May’s – or June’s? If it was reported before, I can’t seem to locate it.
Perhaps the incident will show up in next month’s VA report to Congress? If so, I will update this post with additional details. Or if the VA sees this post and would like to provide additional information, I’ll be happy to post it.
