Medical records for patients in the East Riding have been lost by a firm paid by health bosses to keep them safe in storage.
East Riding Clinical Commissioning Group (CCG) has launched an investigation after discovering paper records had gone missing.

She said: “The storage company commissioned to store case notes and archive case notes was unable to locate a number of records. We have declared a serious incident and are having a good look at what has happened and how we can learn from that.”
A report to the CCG board said the storage firm, which has not been named, launched its own investigation but was unable to find the records or explain where they might be.
